 Basic Document:

  1. Seven (07) months valid passport with old passport if have.
  2. Visiting card.
  3. Recent 2 copy photograph taken in last 3 months (matt paper, white background only)

For Job Holder:

  1. No objection certificate (NOC)
  2. BMDC certificate for Doctor 
  3. BAR council certificate for Advocate
  4. Retirement document for Retired Person

For Business Person:

  1. Renewal trade license copy with notary public ( English Translated )
  2. Memorandum for limited company / Form XII
  3. Blank page of office pad.

For Student:

  1. Student ID card Photo Copy.
  2. Birth certificate (Only for Child & infant).

Others:

Marriage certificate copy ( if spouse name not mentioned in the Passport )
